How do I file a claim for benefits under FEDERAL FIRST's Long Term Disability Insurance Program?

You can select one of three ways to file a claim for LTD benefits

You must give MetLife written Statement of Claim within 30 days after the first day you miss work due to a disability. When MetLife receives your Statement of Claim, you will be sent forms for providing Proof of Loss within 15 days.

My doctor has placed me on disability; what should I do?

File your claim using one of the following methods

If I have a disability claim, is there anything I can do to make the processing of my claim easier?

Yes. Follow these guidelines

  • Complete and sign the Employee Section of the claim's form immediately.
  • Have your employer complete and sign the Employer's section.
  • Mail your form to:

Who do I call for the status of my claim?

Call MetLife toll-free at 866-729-9199, Monday – Friday, 8:00 am – 11:00 pm Eastern Time.

Please Note that if you incurred a claim prior to April 1, 2010, Aetna will service your claim.  To obtain the status of a claim, you will need to call Aetna LTD Claims at: 877-352-3862.

Do I need to wait until after the elimination period to file my claim?

No. A delay in reporting your claim can cause serious delay in payment of benefits.

How long will it take to process my claim?

Disability claims generally take up to 45 days to process.

Will my disability benefit checks be subject to federal income taxes?

No. Your benefits payments will not be subject to federal income taxes since your premiums are paid with after-tax dollars.

How often will I receive a disability benefit check?

Long Term Disability benefits are paid monthly.

Will payment from other sources affect the amount of disability benefits paid to me?

Yes. Payments may be reduced from other sources. Workers’ Compensation, Social Security or the Federal Retirement System benefits are examples.
