Claims
When you are out of work due to a disability, timing is everything to ensure that you receive your benefits without delay.
The most important information to remember when you file a claim
- You must give MetLife written Statement of Claim within 30 days after the first day you miss work due to a disability.
- Do not wait until the end of the elimination period to file a claim; any delay in reporting your claim can result in serious delay in payment of benefits.
- You must continue to make premium payments during your elimination period and while your claim is being processed.

To receive a Statement of Claim form either
- Download a form from our Forms Download page.
- Request one via email using our contact form.
Once MetLife receives your Statement of Claim you will be sent forms for providing Proof of Loss within 15 days. Disability claims generally take up to 45 days to process.
