How do I enroll?

You can enroll online or by sending a completed enrollment form to FEDERAL FIRST Customer Service. Forms can be downloaded from the Forms Download page.

A Direct Deposit Sign-Up Form 1199A will be mailed to you once your enrollment form is processed. Please use this Form 1199A to initiate your allotment through Employee Express, Employee Personal Page (EPP) or submit your signed form to your payroll servicing personnel for processing.

Please note that coverage under this policy will not become effective until premiums are deducted. You’re responsible for making sure the Direct Deposit Sign-Up Form 1199A is successfully implemented and premiums are deducted. Please don’t return the Form 1199A to FEDERAL FIRST; returning your signed Form 1199A to FEDERAL FIRST Customer Service will delay processing of your allotment and may cause your file to be closed.

When you participate in the FEDERAL FIRST Voluntary Term Life Insurance Program, underwritten by MetLife your cost of insurance is paid through allotments while you are an actively-at-work employee .

Do I need to have a physical exam or answer medical questions?

  • You : In $10,000 Increments up to the lesser of $100,000 or 2 times your base annual earnings rounded to the next lower $10,000 increment.
  • Your Spouse and Dependent Children: Up to Option 5 as long as the coverage under the option you elect is not more than the coverage amount you elected for yourself.

Evidence of insurability is required

  • For coverage amounts requested in excess of the amounts shown here if you enroll within your introductory enrollment period.
  • For any amount of coverage you elect for yourself and your spouse if you enroll after your introductory enrollment period.

When will my coverage be effective?

Your coverage is effective the first day of the pay period in which an allotment is taken. In no event will your effective date be earlier than the Group Effective Date or the date you enroll. You must be actively-at-work on the effective date of your coverage; otherwise, your coverage becomes effective when you return to work for at least one day.

If the coverage you elected requires that you provide evidence of insurability, that coverage must be approved by MetLife before a Direct Deposit Sign-Up Form 1199A is mailed to you.

What are considered qualifying events for Family Status Change?

Some examples of qualifying events are marriage, death of spouse, divorce, legal separation, annulment, birth of a child and adoption.

Can I enroll if I experience a qualifying Family Status Change event such as marriage, birth of a child, adoption, etc?

Yes. If you enroll within 31 days of a qualifying life event you may add or increase your coverage by one increment of $10,000. You may also add or increase your spouse and dependent children coverage by one level or option.
