How do I make an allotment change?

Where do I send my signed Form 1199A?

Send your signed Form 1199A to your payroll servicing personnel. If you have access to Employee Express or similar self-service payroll program, you may input/update your allotment information yourself. Do not return your signed Form 1199A to the FEDERAL FIRST Service Center as this will delay processing your allotment.

When do I need to change my allotment?

You need to change your allotment if:

  • You have a birthday that moves you into the next age band.
  • You increase or decrease coverage amounts.
  • You add or change the amount of your spouse and dependent children coverage.
  • You cancel coverage.

What are the allotment/premium age bands?

For active employees

Under age 25 50-54
25-29 55-59
30-34 60-64
35-39 65-69
40-44 70 and above
45-49  

For employees who retire with coverage in-force:

50-64  
65-74  
75 and above  

Does my allotment change if I just had a birthday but I’m in the same age band?

No. Your allotment only adjusts for age when you move from one age band to the next. For example, a participating employee who turns 35 next year will move to the age band including that age. FEDERAL FIRST will send a new Direct Deposit Form 1199A to increase their adjustment.

Does my allotment change if my recent birthday moves me into the next age band?

FEDERAL FIRST will automatically send you a new Direct Deposit Form 1199A along with instructions for processing.

How do I change my premium if my recent birthday moved me to the next age band?

The FEDERAL FIRST Service Center will automatically send you a new Direct Deposit Form 1199A and instructions for processing.
